Do hereby give my/our consent for him/her to participate in the Intro to BJJ seminar. I and my child (ren) fully recognize the risks of injury inherent in participating in any fitness or martial arts program, and I/We represent to the instructors that we have taken all reasonable steps to determine that my child (ren) is in good health and physically capable of participating in the seminar. I/We acknowledge that the instructors shall make no, and shall have no responsibility to make an independent evaluation of my child (ren) physical health or fitness. I/We understand and agree that any participation in the seminars shall be at your own risk.
I/We hereby release,indemnify and hold harmless the instructors from and against any and all claims,and liabilities of any kind or nature to myself or my child (ren). I/We understand and agree that the instructors shall not be responsible for the conduct of others or its facilities,equipment or participants in the seminar. I/We shall not bring any action or proceeding against the instructors for any payment compensation or claim of any injury caused by such user.
